Please bring your
COMPLETED registration forms and your registration fee of $275.00 per student. Make your check payable to St. Matthew Catholic School.
Parents must be prepared to sign the FACTS sheet, sign up
for FACTS if they haven't in the past or pay tuition in full at this time.
Though the office will stock a few re-registration packets for those without computer or Internet access, we would like all other families to download the documents, complete them, and submit them during re-registration in March.
Please do not bring completed packets to the office before re-registration.

- Position your cursor in each of the form fields or tab through the fields (they will show as light yellow in the registration document) and type in your information
- After completing, print out your forms and bring them with you to re-registration

- Universal Medical Information/Emergency Contact form
- Yellow Emergency Release Card (put in Wed. envelope February 27th )
- Medical Release and Consent to Treatment of Child form
- General Terms of Parental Consent form
- Archdiocese Application for Persons Volunteering to Work With Children and Youth
- Electronic Resources and Computer Usage Agreement
- Shield The Vulnerable information
- School Directory Change of Address form - Required for all families
Optional Forms:
- School Counseling Release form
- Website Photo Information form
- CASA Program Contract/Agreement
- Either print out and complete the registration packet by hand or type in your form information online and print out. Use the included checklist to make sure you've completed all the required forms.
- Read the 2008-2009 School Handbook which is posted online prior to re-registration
- Submit your volunteer preferences for 2008-2009 online using the form found at

- Bring the completed packet and your registration fee of $275.00 to re-registration in Cronin Center March 10, 11, 12 and 13. Please make checks payable to St. Matthew Catholic School.
