Re-Registration Checklist
- You MUST read the School Handbook which is posted online prior to registration
- St. Matthew School Family Agreement (one per family)
- Universal Medical Information/Emergency Contact form (each child)
- Blue Emergency Release Card (each child)
- Medical Release and Consent to Treatment of Child form (each child)
- General Terms of Parental Consent form (each child)
- School Counseling Release form (each child)
Please bring your COMPLETED registration forms and your registration fee of $ 300.00 per student, at your assigned time. Make your check payable to St. Matthew Catholic School. You may write one check for all siblings.
Be prepared to pay tuition in full – attach a post dated check (to be deposited July 1, 2010) or bring the confirmation sheet showing that you have registered online with FACTS to pay tuition monthly.
