Re-registration for the 2011-12 School Year

We will conduct re-registration in Cronin Center, Monday, March 14th through Wednesday, March 16th. Times are scheduled according to your last name:

• Monday, March 14th (8:00-9:00 a.m. or 3:00-5:00 p.m. for families whose last name begins with the letters A through H)
• Tuesday, March 15th (8:00-9:00 a.m. or 3:00-5:00 p.m., for families whose last name begins with the letters I through R)
• Wednesday, March 16th (8:00-9:00 a.m. or 3:00-5:00 p.m., for families whose last name begins with the letters S through Z)

On re-registration forms for next year, please be sure that you carefully fill out medical information, birthday...for EACH child.

Re-Registration Checklist

• St. Matthew School Family Agreement (one per family)
• Major Emergency Release Form (one per family)
• Universal Medical Information/Emergency Contact form (each child)
• Medical Release and Consent to Treatment of Child form (each child)
• General Terms of Parental Consent form (each child)
• School Counseling Release form (each child)
• Confirmation Copy that you have signed up online for Yard Duty and Traffic.

Please bring your COMPLETED registration forms and your registration fee of $ 330.00 per student, at your assigned time. Make your check payable to St. Matthew Catholic School. You may write one check for all siblings (excluding Kindergarten - Please keep separate)

If you are paying tuition in full - attach a postdated check (dated June 30, 2011) which will be deposited July 1, 2011.

All families currently on FACTS will be automatically renewed to pay tuition monthly beginning in July, 2011.

If you wish to join FACTS for the 2011-2012 school year, you must sign up and bring the confirmation sheet showing that you have registered online with FACTS to pay tuition monthly.
